FIRST VISIT AT LOWELL STREET DENTAL IMPLANT & SURGICAL CENTER IN SILVERDALE, WA

Greggory Wilde, DDS PS typically sees patients Wednesday through Friday from 8:30 AM to 4:30 PM.

Your first visit will be a thorough evaluation followed by a discussion of our findings. Please allow approximately 30-45 minutes for your initial visit. In order to properly plan your treatment, we typically do not schedule initial exams and treatment on the same day; we want to make sure you have all the information you need, and that all of your questions are answered prior to any treatment being provided. We would like you to be aware that there is a fee associated with your first visit and we are happy to go over those fees with you in advance of your first visit should you desire. Please continue to see your general dentist while undergoing treatment at our office so that your restorative needs continue to be addressed. We are available to respond to your concerns and encourage you to ask questions at any time. We are looking forward to seeing you soon!

1. Registration: Please complete your patient registration forms HERE, or print the forms below and bring them with you to your first appointment. It is important that you list any medications, allergies or other pertinent information that could affect your dental care.

APPOINTMENTS & CANCELLATIONS

We reserve time just for you. Appointments are carefully scheduled so you will be best served. Your prompt arrival will allow us to perform all the treatment that has been scheduled. If you must reschedule, please allow at least 2 business days notice to reschedule a non-surgical appointment and 7 days for a surgical appointment. Cancellation fees in the amount of 50% of the fees to be charged may be added to your account if proper notice is not given.
